Congratualations Grade 7 Qualifiers for S.Y 2023 - 2024
To proceed with the enrollment procedure and secure admission to the University, please submit the following documents starting August 1, 2023 until August 04, 2023 only.

Proceed to Admissions and Orientation Services Office.
- Print out this email this will be serve as your Reservation Form.

Proceed to Infirmary for Medical/Dental Examination then present the following:
- Fill out the Student Health Assessment Form: Click here
- Photocopy of the Official Chest X-ray result
- Medical Certificate, if necessary.
- PWD ID, if necessary.
To complete your pre-enrollment process the following documents must be submitted at the University Registrar office on or before August 04, 2023 only.
- One (1) piece 2 x 2 picture in white background.
- Original Copy of Certificate of Good Moral Character.
- Original Copy of Grade 6 Report Card SF 9 (previously Form 138).
- Photocopy of PSA Birth Certificate.
Please place all documentary requirements in a long brown envelope and affix an appropriate label on the back containing the following information:
- Name: Juan A. Dela Cruz
- Course: Grade 7 Regular / Grade 7 S.T.E
- Applicant No.: 2023000001
Once admitted, the Office of the University Registrar will send your student number and default password to your registered email address.
Furthermore, the enrollment schedule will be announced and posted on the student portal.
Important Note:
To request an SF 10 (previously Form 137), qualifiers must download and fill out the Request Form for SF 10 (previously Form 137) from the following link: Click here
The deadline for submission of SF 10 (previously Form 137) is on November 15, 2023.
Please note that any attempt to submit documents after the specified deadline will not be accommodated.
